Form SS1  -   A Health & Safety Snapshot

Provides a straightforward description of who the new three duty holders are, under the new Health & Safety law.  This should help you identify where you fit in;  it includes, PCBUs (Person Conducting a Business or Undertaking), Officers and Workers duties and outlines what their duties are. 

It also provides an overview of 11 key steps for implementing and maintaining an active health and safety system.

 

Form SS2  -  Health & Safety Policy and Commitment Statements

A quick and easy description of what these documents are and their purpose.  It includes examples of a business Commitment Statement and a Health & Safety Policy that can be used to model your own statement and/or policy, to best fit your business. 

A Health and Safety Policy provides evidence of your business’s intentions to manage health and safety.

 

Guide SS3  -  Health & Safety Hazard and Risk Assessment Guide

This Guide explains the difference between hazards and risks.  It includes key information about:  finding hazards, involving your workers in the process, examples of typical hazards and a risk management process. 

A description of a risk assessment process is also included together with an extensive hazards and controls checklist that should cover most aspects of any workplace.

 

Plan SS4   -   Health & Safety Plan

A Health & Safety Plan template is provided with ready to use documents which enable you to record health and safety objectives, actions, timelines and budgets.

Activities that ensure an effective health and safety system are included, such as: employer commitment, planning and review, hazard identification and risk management, accident/incident recording and investigation, worker participation and emergency planning.  This is a significant "must have" and is easy to follow and use.

 

Form SS5  -  Safety in Seconds – safety briefing example and worksheets

This is a template for you to use for recording details of safety briefings including the time, date, subject covered, issues raised and who attended.  It also provides examples of topics to discuss that will be essential for your businesses health and safety. 

We've taken the hard work out of it for you, you just need to hold the meetings and record the event.

 

Form SS6  -  What should be in a Site Specific Safety Plan (SSSP) - you will need this information if you go out on a site to undertake work OR  if you engage any subcontractor to work on any site

These plans are widely used in the construction and other related industries OR where activities are project based.  This document describes the components of what should be in a SSSP.  It also lists the relevant Smart Start H&S 12 Pack forms to use, to enable you to quickly assemble  your own SSSP if required.

This form will also let you know what forms  all subcontractors  working on your site should have. 

You will need to ensure all subcontractors have the right documentation and are performing their health & safety, whilst on your site or at your workplace - ask to see their SSSP.

 

Form SS7  -   Managing Hazards and Risks

This  Guide provides the information and advice on “how to” identify and manage hazards and risks.  Managing risks is at the heart of your health and safety system and involves 5 Steps of : identify hazards,  deciding who might be harmed and how, assessing the risk and deciding on precautions and take actions.  It also includes what you will need to record your assessments and review what actions you to take to ensure everybody remains safe.

The Guide includes a useful list of common hazard types, an action flow chart and practical examples of controlling hazards.

 

Guide & Registers SS8   -  Hazard and Risk Assessments and Guide and Registers   FREE

These documents guide you through the process of assessing risks from the hazards you have identified.  An easy to follow ‘Risk Matrix’ provides a quick and easy way to determine how serious the risk is and aids you in deciding what action to take for each hazard.

The document includes two very important registers that support your health and safety system:   A Hazard Register  and a  Risk Assessment Register.

 

Registers SS9  -  Hazardous Substances (Dangerous Goods Register and Maintenance Register

This document gives you to two registers that will support your health and safety system – especially if your businesses uses or stores hazardous substances.   

The Hazardous Substances register contains the type of information that is necessary to manage the risks.  The Maintenance Register enables tracking of the type and extent of equipment (including machinery) and plant used by your business and how and when it has been maintained to ensure safety. 

 

Worksheet  SS10  -   Job Safety Task Analysis  (JSTA) Worksheet

Understanding the hazards and risks involved with the various tasks your team undertakes is critical to successfully managing health and safety.   This document provides 6 key  “how to” steps to undertake a Job Safety Task Analysis (JSTA).   It includes a helpful worksheet that makes the process easy.   

The worksheet sets out the hazards and controls for each job and who is implementing the required actions -  An easy to follow format.

 

Form SS11 -  Pre Start-Up and Start-Up Checklist

These checklists contain all the items you need to check on and make sure are covered before work starts on a new project, activity or site.  

The checklist are particularly helpful when starting up new physical worksites   eg  building, construction or when a new process is being put in place, within an existing workplace.

 

Plan SS12  -  Emergency Health and Safety Plan

This document provides you with an Emergency Plan template which includes the necessary actions, procedures, first aid and treatment information and allocation of responsibilities.  An example of a Health and Safety Plan is provided which includes emergency actions and information.   We have also included an easy to use template for you to fill in and use in your business. 

A key feature included with this, is an Emergency and Evacuation Plan that your business can easily complete and post as your 'Emergency and Evacuation Plan notice' in your workplace.

 

Form SS13 -  Accidents and Incidents Packs

This Pack provides all the Information on what actions to take in the event of an accident or incident.   It also includes, what information to record and notification to give, in the event of an accident or incident.   Importantly, this includes what information is required for notification of accidents or incidents to WorkSafe NZ. 

This is a very helpful template to use to assist you with your own accident investigation should you need it and a guides you through what process to follow.

 

Register  SS14  -  Training and Skills (Competency) Supervision Register   FREE

This register provides a template for your business to use for recording and reviewing critical information on the safety skills, training and supervision of your workers in relation to the work or tasks they are undertaking.


Registers SS15   -  Site Induction and Site Registers   FREE

This document provides two registers to support your health and safety system.  The 'Site Induction Register' records and confirms details of those who have received induction safety training before they commence work at your site or project.  The ‘Site Register’ records information on the contractors or subcontractors that are operating on your site or premises.  

This information is extremely important as all businesses  will be required by law to coordinate safety arrangements where they share / work on, the same site.
